Regular Health Monitoring – What Tests You Need and When

Quick Overview #

Being stable on treatment does not mean you stop monitoring. Regular check-ups catch problems early, confirm your treatment is working, and keep you in the best possible health. Here is what to expect and when.

Why This Matters #

When you are feeling healthy and your viral load is undetectable, it is easy to think “why do I still need to go to the clinic?” The answer is simple: monitoring keeps you healthy. It catches potential problems before they become serious, confirms your medication is doing its job, and screens for other health conditions that people living with HIV should watch for.

Dream Village’s CATS team supports this by helping 2,896 young people stay connected to their care in Q1 2025 alone, with 95.5% maintaining a valid viral load result on record. That kind of consistency does not happen by accident. It happens through regular engagement with the health system.

Viral Load Testing #

This is your most important routine test. It measures how much HIV is in your blood. If your viral load is undetectable (below 50 copies per milliliter on most tests), your treatment is working perfectly.

Schedule: Typically tested at 3 months and 6 months after starting treatment, then every 6 to 12 months once stable. Your provider may test more frequently if there are concerns.

What to watch for: A detectable viral load after previously being undetectable could indicate adherence issues, drug resistance, or drug interactions. It does not necessarily mean treatment failure, but it warrants a conversation with your doctor.

CD4 Count Testing #

Your CD4 count measures the strength of your immune system. Once you are stable on treatment and your viral load is undetectable, your CD4 count typically recovers and stabilizes.

Schedule: Often tested alongside viral load in the first year. Once stable and above 500, testing may become less frequent, perhaps annually.

What to watch for: A declining CD4 count alongside a suppressed viral load is unusual and may indicate other health factors worth investigating.

TB Screening #

Tuberculosis remains a significant concern for people living with HIV, especially in sub-Saharan Africa. Regular TB screening is part of standard care. If you develop a persistent cough, unexplained weight loss, night sweats, or fever, get tested for TB promptly.

Your health facility may also offer TB preventive therapy (TPT), which significantly reduces your risk of developing active TB.

Other Important Screenings #

STI testing. If you are sexually active, regular screening for sexually transmitted infections (every 6 to 12 months, or sooner if you have new partners) is important.

Liver and kidney function. Some ARVs can affect liver and kidney function over time. Blood tests to check these organ systems are typically done annually.

Cervical cancer screening. Women living with HIV have a higher risk of cervical cancer. Regular Pap smears or HPV screening should be part of your care, starting at age 25 or earlier if your provider recommends it.

Mental health screening. Depression and anxiety should be assessed at regular intervals, not just when you report symptoms.

Preparing for Your Appointments #

Write down your questions before you go. Note any changes in how you feel, even if they seem minor. Bring a list of any other medications or supplements you are taking. Be honest about adherence. Your medical team is there to help, not to judge.
